Gautier Middle School

All band students need to have a Dragonfly Max Account. If your child has a Dragonfly Max account from last year, you will just need to update the paperwork.  You will need to remember your login to do this.  If you are new to this process, you will need to follow the instructions below and follow this link: Dragonfly for Gautier Middle School

Please remember that band students are not required to have a physical.  They must have all other paperwork completed and signed electronically. The following forms are required to be electronically signed and submitted:

1. Electronic Signature Agreement
2. MHSAA Student Participation Clearance Form
3. PGSD Drug Policy Form
4. Consent to the Disclosure and Use of Personal Health Information Form
